Required Forms for all Grants
Below please find the five required forms for most grants managed by our agency.
W-9 Form (PDF)
Completion of the IRS W-9 form is required for each agreement; including exempt entities. We are required to withhold twenty percent (20%) of any payment to an individual or company for which we do not have either a social security or a Federal Tax I.D. Number. If you believe payments to you by the WMD are exempt from the IRS reporting requirements, please provide an explanation of the exemption, with reference to the appropriate IRS regulations providing for such exemptions.
Signature Authorization Form (PDF)
Identifies the persons in your organization who have authority to sign grant agreements, amendments, and requests for reimbursements.
Debarment Certification Form (PDF)
This certifies that your organization will not use funding from this grant agreement to do business with any person or organization debarred from doing business with the U.S. government.
Federal Funding Accountability and Transparency Act (FFATA) Form (PDF)
This worksheet is required for any recipient of federal funds equal to, or in excess of $25,000. If an award is below this amount initially yet later increased, the act is triggered. Due to this variability in compliance the Military Department requires this form to be filled out for each grant agreement. If your organization does not meet the criteria in section E, specifically identify each of the sub sections of (E) numbers 1 or 2 or both that is not met for your organization.
2 CFR Part 200 Subpart F Audit Certification Form (PDF)
Indicates whether you are subject to the requirements of OMB Circular A-133 and if you are, certifies that you are complying with the audit requirements within the Circular.
Required for ONLY U.S. DOE & EFSEC grants
Energy Facility Site Evaluation Council grants: Funds to conduct radiological emergency preparedness programs relating to the Columbia Generating Station nuclear power plant.
U.S Department of Energy grants: Funds to conduct and maintain emergency preparedness programs relating to the Hanford Site.
